New Medicare Program Pays PCPs $40 Per Month Per Patient


...for Chronic Care Management Outside of Office Visits.

Previously we talked about a new program offered by CMS which provides additional revenue to Primary Care Physicians. The new code is included in the CMS fee schedule published November 13. The program will pay $40.00 per month to provide cronic care management to Medicare patients with 2 or more chronic conditions.

New Code to Bill Medicare Released Nov 13

In order to bill Medicare for this service you must have a letter signed by your patient designating you as the patients chronic management care provider. We can print the letters at your office from your Medisoft Clinical database. Our program automatically identified your patients that qualify and will print the letters for you to mail to your patients. Contact our office for more information.
